Community Nursing Care and Support for Patients in Their Homes

Overview

Community nursing teams offers a range of services to adults who are
unable to access community clinics. This service is designed for housebound
individuals in the NHS Mid Essex area.

Eligibility Criteria

•  Adults registered with a GP in the NHS Mid Essex area.

•  Must be housebound.

This is someone who is unable to leave their home environment
due to a physical or psychological illness. You are not housebound if you are
able to leave your home environment with minimal assistance from others eg
family, friends, or carers to attend the doctor, dentist, hairdresser or
leisure venues. You will be individually assessed by a nurse to determine your
eligibility for home nursing visits, this will be assessed on individual need
and clinical judgement and will be discussed with you. As your health improves
and you are no longer housebound, we will require you to attend GP practice or
clinic settings. 

Service Details

Our treatments include:
•  Wound management for trauma and chronic wounds.
•  Lower-leg care for various types of ulcerations.
•  Post-operative care, transitioning to GP Practice nurses once ambulant.
•  Administration of medication by injection (excluding oral medications and eye drops).
•  Management of urinary catheters and bowel issues.
•  End of life or palliative care.
•  Supporting self-care for patients and carers, including administering injections, PICC lines, eye drops, and dressings.

Our Team

Our team consists of nurses with a wide range of skills and knowledge, who have had additional specialist training. They provide care to patients who have specific needs which can be met by the specialist skills of the community nurses.

Service Delivery

•  Care is provided exclusively to housebound patients.
•  We operating a day service: 8am to 8pm, and an out of hours service between 8pm-8am Monday to Sunday including Bank Holidays.  
•  For assistance hours team between 11pm and 8am please contact 111. 

Referral Process

Referrals can come from a variety of sources including self, GP and hospital.

Contact Us

If you are registered with SystmOnline you will be able to view your own Electronic Medical Record (EMR), manage your appointments, request repeat medication, update contact details and message your healthcare service. There is also a SystmOnline app available for Apple and Android devices.
