Adult Community Occupational Therapy and Physiotherapy Service

Tailored rehabilitation service to help people in the community. 

Overview

Our team comprises of Physiotherapists, Occupational Therapists and Therapy Assistants. We offer assessment, advice and tailored rehabilitation to people who have a health-related illness, injury or long term condition to help recover their health, cope with chronic conditions and slow the progress of degenerative disease. 

We deliver a time limited course of rehabilitation to reach agreed personalised goals to improve independence and safety with everyday activities. This may include the provision of a home exercise programme, the provision of adaptive equipment, advice and education around a condition and signposting to help support you in managing your own condition. 

We recognise that people can have multiple needs and several services and disciplines working collaboratively to meet these. As a service we work in partnership with our community colleagues in health and social care for the best outcome for the individual and to enable them to reach their goals. 

The service aims to :

  • Maximise independence at home.
  • Improve quality of life. 
  • Prevent hospital or care home admission. 
  • Support discharge home from a hospital or care home.

Who Can Access This Service?

Criteria

  • Adults (18+) registered with a NHS Mid Essex GP. 
  • People who are housebound, those diagnosed with a neurological disorder may be treated in an outpatient setting if clinically more appropriate.

Inclusion: People who require

  • Requires rehabilitation to regain or improve independence and safety with mobility, movement or other everyday activities.
  • Advice and education to promote self-management of a long term condition and reduce the risk of secondary complications.
  • Therapy intervention to help slow the loss of function for those with progressive conditions. 

Exclusions: People who

  • Require an urgent response to prevent an admission to hospital should be referred to Urgent Community Response Team (UCRT) on 0344 257 3951.
  • Can access the Outpatient Musculoskeletal Physiotherapy Service.
  • Require provision of long term equipment (over six months) or major adaptation should be referred to the Social Care Occupational Therapy team on 0345 603 7630 socialcaredirect@essex.gov.uk. 
  • Are under the care of a Reablement services. 
  • Requires specialist services (Stroke, Learning Disabilities, Hospice).
  • Have a neurological diagnosis with a CM1 postcode and requires outpatient neurological physiotherapy should be referred to MEHT outpatient therapy service. 
  • Experience dizziness without a diagnosed neurological cause. 
  • Have been discharged from our service in the last six months and whose condition has not changed.

Service delivery 

The service operates Monday to Friday, 8.30am – 4.30pm delivering rehabilitation to people in their normal place of residence for example home, care home or supported living environment. 

For people with a diagnosed neurological disorder, we will treat them in the most appropriate setting which could be at home or an outpatient clinic.

Treatment could include a mixture of face to face and virtual interventions.  

Referral Process

You must be referred by a Health or Social Care professional, such as your GP or Social Worker.

All referrals are triaged by a clinician. If the referral is accepted it will be added to a waiting list and contact made when an appointment becomes available.

If you are registered with SystmOnline you will be able to view your own Electronic Medical Record (EMR), manage your appointments, request repeat medication, update contact details and message your healthcare service. There is also a SystmOnline app available for Apple and Android devices.
