Overview
Empowering Heart Health is a specialized service dedicated to providing comprehensive care and education for individuals diagnosed with Angina or managing heart failure. This service is tailored to adults registered with a GP in the NHS Mid Essex area, meeting specific criteria set by the Clinical Commissioning Group (CCG).
Who Can Benefit from This Service?
Our Cardiac Service is designed for individuals who have been diagnosed with Angina or are managing heart failure.
Eligibility criteria
- Adults registered with a GP in the NHS Mid Essex area.
- Meet specific criteria set by the CCG, as advised by your healthcare professional.
Our Service
For Angina Patients:
“The Angina Plan” – A twelve week self-management plan providing support, education, and exercises that can be performed at home or in our cardiac rehabilitation classes.
For Heart Failure Patients:
- Long-term medical management from diagnosis to end-stage.
- Medication optimisation to control symptoms and provide appropriate treatment.
Treatments and Services
- Clinic and home visits for assessment, education, and medication optimization.
- Inpatient hospital visits for pre-discharge assessments.
- SOS response for urgent assessments to prevent hospital visits.
- Comprehensive cardiac rehabilitation – an eight week tailored exercise program with condition management education.
- Clinical assessments including blood pressure, pulse oxygen saturation, and venepuncture.
- ECG heart monitoring, including 24-hour ambulatory monitoring.
- 24-hour ambulatory blood pressure monitoring.
- Cardiac Rehabilitation classes.

Specialist nurses, healthcare assistants, and Cardiology GP with Special Interest (GPwSI).
Our Locations
Operating Hours: Monday to Friday, 9am-5pm.
Locations: Broomfield Hospital, Halstead Hospital, St Michael’s Health Centre, St Peter’s Hospital, and Burnham Clinic.
Note: Appointments must be booked through the Cardiac Service, not the venue.
Home and hospital visits as required.
How to Access Our Service
Referral Process:
- Referrals are made by a GP or healthcare professional.
- Existing patients can contact us for information and advice post-discharge.

If you are registered with SystmOnline you will be able to view your own Electronic Medical Record (EMR), manage your appointments, request repeat medication, update contact details and message your healthcare service. There is also a SystmOnline app available for Apple and Android devices.
