Community Wards (inc Community Stoke Rehabilitation Unit)

Bayman Ward

Service Overview:

Bayman Ward is a 25-bed stroke rehabilitation ward at Brentwood Community Hospital. Bayman Ward is a nurse led unit and patients are admitted onto a 6-week stroke pathway for stroke rehabilitation, the discharge date is set on admission. Patients are reviewed and discussed at weekly MDTs this comprises of Physiotherapists, Occupational Therapists, Speech and Language Therapists and Psychologists, stroke consultant and nurses who all deliver support tailored to the patients’ needs. Our therapists work with patients to set their own goals.  Patients receive an initial pack welcome pack on arrival to welcome them to the ward which provides information about the ward and the pathway.

Patients who require stroke rehabilitation are transferred to us from a variety of hospitals including Basildon, Colchester, Queens and Broomfield Hospitals.

Patients admitted to Bayman ward will be supported to reach their optimal level of independence enabling them to return home or into interim placement depending on their individual needs. This will be discussed at a discharge meeting at around week 3 of the pathway where patients and family can discuss with therapy the progress made and what support will be required for a safe discharge from Bayman ward.

Patient will be supported with a package of care if needed and early supported discharge which provides ongoing therapy in their own homes or interim placement.

When a patient is discharged, we complete a survey about their experience on the ward. However, patients can also complete our feedback survey (known and the Friends and Family Test) by clicking here. Or contacting customer services.
