Adult Diabetes Service

Specialised Care for Managing Diabetes

Overview

Our Diabetes Service offers specialised care for adults (18+) with Type 2 diabetes and some with Type 1 diabetes, focusing on helping them to self-manage their condition.

Eligibility Criteria

Registered with a GP in the NHS Mid Essex area, patients can access this service via a referral from the GP surgery.  Those who are pregnant, need pre-conceptual counselling, are newly diagnosed with type 1 diabetes, have multiple diabetes-related complications, or require an insulin pump, can be supported by the MSE Team at Broomfield Hospital.

Service Details

We cater to the needs of people with type 2 diabetes and some patients with type 1 diabetes by offering:

  • Glycaemic intensification.
  • Insulin conversion.
    Use of Continuous Glucose Monitoring (GMG) for those who meet locally set prescribing guidelines.
    Initiation of GLP1’s receptors.
    Telephone and email support between appointments to optimise glycaemic control.
  • Structured group education for type 2 diabetes, individual sessions or access to MyDiabetes APP
  • Smooth transition and quick discharge from Mid and South Essex (MSE) hospitals.
  • Home visits for housebound patients.

Virtual support for those who live in a Nursing Home or Care Home setting.

Our multidisciplinary team consists of Diabetes Nurse Specialists and Dietitian.

Service Delivery

Clinics are available at various locations:

  • Moulsham Lodge Clinic ,Chelmsford
  • Braintree LiveWell Hub
  • St Peter’s Hospital, Maldon

Educational sessions are also offered in Chelmsford, Witham and Braintree

Referral Process

Referrals must be made by a healthcare professional, such as your GP, MSE diabetes centre, or other community services.

For our education programme, you may self-refer.

For your first appointment, please bring with you a list of your current medications, your blood glucose meter and diary if you are self-testing and any diabetes-related records. The team aims to see patients within 18 weeks of referral.

MyDiabetes APP

The MyDiabetes APP is available for those with Type 2 Diabetes, offering comprehensive information and guidance, including DMFit, diet and exercise tips, understanding HBA1c results, and foot and eye care advice. If you are registered with SystmOnline you will be able to view your own Electronic Medical Record (EMR), manage your appointments, request repeat medication, update contact details and message your healthcare service. There is also a SystmOnline app available for Apple and Android devices.
