Urgent Community Response Team (UCRT)

Rapid, Compassionate Care in the Comfort of Your Home

Overview

The Urgent Community Response Team (UCRT) is a national initiative that provide urgent care to people in their homes which helps to avoid hospital admissions and enable people to live independently for longer. Our dedicated team will address the urgent needs of patients in their own homes or care settings.  The team will attend the patient within 2 hours of referral.

Our Team compromises of senior clinicians, non-medical prescribers, Occupational Therapists, Physiotherapists and Health Care Support Workers. This multidisciplinary team is adept at ensuring patients can remain in their familiar surroundings during acute illness, by performing a holistic assessment and develop a plan of care in conjunction with the patient and relatives.

Once a plan of care has been agreed the team may refer to other specialty services to continue to monitor the patient until they have recovered from there acute illness or discharge back to the care of the GP.

UCRT has access to a Frailty Consultant for those more complex patients for advise and support when devising a appropriate plan of care.

UCRT have therapy staff that will assess adults that are experiencing a sudden decline in mobility or function and are able to order equipment and home adaptations to optimise functional ability and safety at home.

Inclusion Criteria:

Patients registered with a GP in the Mid Essex area including temporary residents.

Conditions such as simple infections, minor illnesses, urgent medical needs, falls, frailty, simple trauma wounds, non-emergency respiratory issues, exacerbations of chronic conditions, non-injury falls, urgent diagnostic blood tests, non-routine catheter management, urgent equipment needs

Exclusion Criteria:

Individuals under 18 years of age.

Cases of acute medical emergency (e.g., sepsis).

Falls with suspected fractures, reduced consciousness, suspected head trauma, especially if on anticoagulant therapy.
